Greta Evans, an American hailing from Montana, embarks on a journey to the United Kingdom upon securing a position as a nanny for the elderly Heelshires. Upon her arrival, the couple acquaints Greta with her unusual charge: a life-sized porcelain doll named Brahms, treated as their son. The Heelshires provide Greta with explicit instructions, including reading to Brahms and playing loud music for him, as they head off on holiday.


Initially disregarding the rules, Greta maintains regular contact with her sister, who reveals that Greta's abusive ex-boyfriend, Cole, is attempting to locate her. The local deliveryman, Malcolm, becomes a frequent visitor, and Greta discovers that the real Brahms perished in a fire two decades earlier on his eighth birthday. As Greta prepares for a date with Malcolm, strange occurrences unfold, including the disappearance of her dress and jewelry, leading her to the attic where she finds unsettling photographs of young Brahms.


Intrigued by Malcolm's insights into the peculiar nature of the real Brahms, Greta becomes increasingly aware of mysterious events, such as the doll seemingly moving on its own and eerie sounds echoing through the hallways. A phone call from a child's voice urging her to adhere to the rules prompts Greta to lock herself in her room. An offering of her favorite peanut butter and jelly sandwich outside her door convinces her that Brahms' spirit resides within the doll, compelling her to take the rules seriously.


Realizing that the doll only moves when absent, Greta demonstrates this to Malcolm. He discloses a dark incident from Brahms' past involving the death of a girl in the forest, leading to the burning of the Heelshires' house with Brahms inside. Despite Malcolm's warnings, Greta, haunted by her past miscarriage at the hands of Cole, feels compelled to care for Brahms. Meanwhile, the Heelshires write a farewell letter to Brahms before tragically ending their own lives.


As tensions rise, Cole arrives to coerce Greta to return home. Concerned for her safety, Malcolm stays close by. In a desperate plea for help, Greta communicates with the doll. Cole discovers a blood-written message instructing him to leave, leading him to destroy the doll. The house shudders, and the truth emerges as the real, now-adult Brahms, donning a porcelain mask identical to the doll's face, emerges from behind a shattered mirror. Brahms dispatches Cole and turns his attention to Malcolm and Greta.


In their attempt to flee, Greta and Malcolm stumble upon Brahms' hidden room, where a doll crafted from Greta's belongings lies in Brahms' bed. The Heelshires' goodbye letter reveals their plan to abandon Greta as a mate for Brahms. Brahms incapacitates Malcolm, threatening harm if Greta leaves. Armed with a screwdriver, she enforces the rules, forcing Brahms to bed. Brahms requests a good night kiss, but Greta defends herself by stabbing him. In a struggle, she prevails, rescuing Malcolm, and they escape the house. The aftermath sees someone repairing the doll on the stairs.
